If your chapter is hosting in-person events, and venues require proof of insurance, please use the documents below:
If the venue/campus is asking to be added to the certificate of insurance for the dates of the event, we can do that as well. Just send the following info to Chapters@csteachers.org, I'll relay it to our insurance folx, and we'll get the certificate set up for y'all:
- Name of event: i.e. CSTA Chapter Palooza
- Venue address: i.e 123 Sesame St, Rancho Cordova, CA 95670
- Date(s) of event: i.e Aug 1 - 5, 2023
- Additional insured: (name to add to the certificate): i.e. University of Wyoming
